1. PASSPORT
1. Namibian Full Birth Certificate or (Citizenship Certificate-persons born outside Namibia)
2. Namibian ID
3. Marriage Certificate (Married women only)
4. One passport photo

- CHILDREN UNDER AGE 0-15
1. Birth Certificate or (Citizenship Certificate - persons born outside Namibia)
2. One passport photo
3. Parents Documents e.g. Birth or Citizenship Certificates or ID
4. Applicant’s signature (14 years old and above)

- EMERGENCY TRAVEL CERTIFICATE
1. Namibian Birth Certificate or (Citizenship Certificate- persons born outside Namibia)
2. Two passport photos
3. ID copy or proof of ID (16 years and above)
4. Marriage Certificate (Married women only)
5. Parents Documents e.g. Birth or Citizenship Certificates or ID (applicants under the age of 16)
6. Applicant’s signature (14 years and above)
NB: A COMMISSIONER OF OATHS MUST CERTIFY THE APPLICATION
